Title: Post Acute Coordinator - Care Management

Location: Maury Regional Medical Center

Position Shift: Full Time 8a-430p

Position Summary:

The Post Acute Coordinator works directly with the care manager and nurse navigator in daily discharge planning functions. This includes assisting with the provision of patient choice for post-acute services and ensuring these choices and referrals are well documented in the EHR. The position assists in the delivery of required regulatory notices.

Credentials / Education:

* LPN Preferred

* High School Diploma or Equivalent Preferred

* 2 Years Med/Surg Experience Preferred

* 2 Years Home Health, Community Health Services, or Discharge Planning Preferred
