Job Description:

This role is responsible for conducting community outreach to recruit and engage at-risk fathers. Roles include: community outreach specialist, facilitator, and community health educator.

ESSENTIAL DUTIES & RESPONSIBILITIES

  • Lead community outreach efforts in order to reach and engage fathers in need of parenting education: Wayne, Moore, Marshall, Jackson, Fentress, Warren, and Van Buren.;
  • Implement and facilitate the fatherhood curriculum, 24/7 Dad AM, in the home and in group settings;
  • Conduct health education and promotion activities
  • Assist in the development of partnerships with other community-serving organizations Provide case management services, health promotion/education and parenting education.
  • Maintain clinical documentation in an electronic health record.
  • Meet all required deadlines for documentation.
  • Demonstrate cultural awareness and sensitivity in working with families from diverse backgrounds.
  • Provide in-home and virtual care coordination for program participants.
  • Attends meetings promptly and actively participates in: trainings, staff meetings, clinical supervision
  • Attend community meetings and events as directed including health councils and community advisory boards.
  • Conduct outreach activities/groups/classes in order to educate community partners/agencies/participants.
  • Staff is familiar with and makes use of the community and resources, both formal and informal within the community and accessing them first before looking to formal resources
  • All must be done within a standard 40 hour week
  • Other duties as assigned by Program Manager and Project Director

KNOWLEDGE, SKILLS & ABILITIES

Knowledge of MCH and fatherhood populations. Able to create a culture of respect. Committed to the program principles. Skilled in networking among agencies and individuals.

MINIMUM QUALIFICATIONS

Minimum Education Level

Bachelor's degree in social work, psychology, or a related field preferred and 1 year of experience in community outreach and/or working with at-risk fathers/families. Bilingual preferred. Must have reliable transportation, liability insurance, and TN driver's license.

Entry Pay Range:

$15.11--$18.84
