Title: Clinical Case Supervisor - RN - Home Health

Location: Maury Regional Medical Center

Position Shift: FullTime Days

Position Summary:

Responsible for care coordination and the provision of effective and efficient clinical services provided by the visiting staff to home care patients. Directs care of patients in their place of residence as well as provides care as needed. The role is to focus on care planning, utilization of services and staff support. Must possess a good working knowledge of the nursing process and or therapy and demonstrate clinical competency and leadership.

Credentials / Education:

* Current and Valid Licensure as a Registered Nurse Required

* 5 Years Clinical Nursing Experience Required

* 2 Years Home Health Experience Required

* 2 Years Management Experience Preferred

* High School Diploma or Equivalent Preferred
